Is there a framework to decide if something really is drug repurposing and requires a predictable business case eg with cost-based plus pricing? We’ll need cut off criteria to prevent ever greening

I think you need to answer 2 questions to determine if you want to pay a higher price than the generic price for an old repurposed drug. First, is it something we want on-label? criteria may be whether it is substantially different from the original indication and that is is not well-established. Second, is generic competition really the issue? E.g. are generics on the market that cause the problem that you can not earn back the investment. If the answer is yes to both questions, I would be willing to pay a higher price. And what that higher price should be > maybe first pay the list price instead of discounted prices. and if still a higher price is needed, cost-based pricing
